Amend a DOC


This form is for use ONLY to request that ODVA Amend and Restate an ODVA Declaration of Conformity (DOC), which is currently valid for products listed on the DOC, for a change in the Minor Revision of the product, as defined by the CIP Identity Object, Attribute 4, which has zero impact to the CIP and network functionality in the device and as allowed in ODVA’s Policy Regarding Compliant Products.
